- Accident Claim Form
This form is to be completed by any student injured in a varsity, club, or intramural sport within 90 days of injury and returned to the athletic director, West Gym 207. Please note that Carleton College provides supplemental insurance only. All injuries must be reported on the Accident Claim Form. Bills are to be submitted to the student's primary health insurance first and then to the PEAR department's supplemental insurance provider.

ALL RETURNING student-athletes must fill out this form each year to participate in varsity athletics.

- NCAA Eligibility Form
ALL student-athletes must fill out this form each year to participate in varsity athletics. The athletic director will hold a group meeting at the beginning of each varsity sport so only complete this form if you missed the meeting.

List of NCAA banned drugs.
- Pre-Participation Physical Form for First-Season Athletes
ALL FIRST-SEASON student-athletes must have a physician complete this form (pages 1,2, and 6 only; we do not need the immunization pages.) First-year students must complete all 6 pages.
- Sports Information Form
ALL student-athletes should fill out this form prior to each season if they want personal information updated on the website. Information provided will be made public.
