New Course Proposal
After completing this forms online, you must print a copy and obtain the necessary signature(s) from department chair(s) but you may begin the process by electronically sending the entered information to the Office of the Registrar. Once the printed copy has been signed please send it to the Registrar.RAD Designation Worksheet
This is both a worksheet to check and see if you have a RAD course as well as an approval form. Submit online to process the information.
