Students on Carleton programs, DIS, and NUS are required to complete a Health Assessment Form. The form can be accessed here. Health Assessments may be done by any licensed medical provider. Most students see their home doctor, or go to the local Allina Clinic or Family Health Medical Clinic. A special health assessment clinic is held at Carleton College each term to which eligible students will receive an invitation to schedule an appointment. 

The Health Assessment form is due the third Friday of the term prior to the program. For 2011-12 the deadlines are:

Friday, September 30, 2011 for winter term 2012 programs

Friday, January 20, 2012 for spring term 2012 programs

Friday, April 13, 2012 for summer/fall 2012 and academic-year 2012-13

Most non-Carleton programs also have a health screening process. Look for details in your acceptance packet.
