Students on Carleton programs, DIS, and NUS are required to complete a Health Assessment Form. The form can be accessed here. Health Assessments may be done by any licensed medical provider. Most students see their home doctor, or go to the local Allina Clinic or Family Health Medical Clinic. A special health assessment clinic is held at Carleton College each term to which eligible students will receive an invitation to schedule an appointment. 

The Health Assessment form is due the third Friday of the term prior to the program. For 2012-13 the deadlines are:

Friday, September 28, 2012 for winter break 2012 and winter term 2013 programs

Friday, January 18, 2013 for spring term 2013 programs

Friday, April 19, 2013 for summer/fall 2013 and academic-year 2013-14

Most non-Carleton programs also have a health screening process. Look for details in your acceptance packet.
