Passport Day

On-Campus Passport Processing

Need a new passport? Join us for on-campus passport processing on Wednesday, April 4, 9-11:30 AM in the Sayles-Hill 2nd floor atrium area.

You need a new passport if you:

(a)  Have never had a passport
(b)  Got your current passport before you were 16 years old
(c)  Have a passport that was issued 15+ years ago
(d)  Lost your passport

To apply for your new passport on campus, bring:

(a)  An application Form DS-11 (download from http://travel.state.gov/passport/). NOTE: You must sign the application in the presence of the Rice County Recorder, so do NOT sign this ahead of time;
(b)  A certified copy of your birth certificate from the County or State level (NOT from the hospital level);
(c)  A passport photo that meets federal passport requirements OR $10 to have your photo taken on-site;
(d)  A valid driver’s license;
(e)  A photocopy of your I.D. (Photocopy must be on plain white, 8 1/2 x 11" standard paper stock, showing the front and the back of your I.D. The photocopy must contain images on only one-side of each page submitted);
(f)   A check for $110.00 made out to the US Dept of State
(e)  $25.00 cash or check for the Rice County Recorder
