Smarter logic around rewrites and url history updates

The page content manager is a little smarter about URL history updates and rewrites.

Specifically:

1. URL rewrites and URL history updates are triggered only when a page url_fragment or parent changes.
2. URL rewrites can now be triggered (as appropriate) by save and continue, save and associate, or save and finish actions.
