"No man is an island," wrote John Donne. That's certainly true at Carleton, where we are committed to providing students with a full range of support services for their physical and mental health, wellness, accessibility, spiritual practices and other needs.

Health Insurance
We require all enrolled students to have health insurance, either under your family's policy or purchased through the College.

MyStudentBody.com Alcohol
Complete the self-administered risk assessment based on a student's alcohol-related beliefs, behaviors, and experiences. All incoming students are expected to visit the site and the complete the first-year curriculum prior to arriving on campus this fall.

Medical History Form
To help us provide quality health care to Carleton students, we ask that you take time to thoroughly complete the Report of Medical History Form. This required form is due by August 6, 2008.

Disability Self-Disclosure Form
To receive academic accommodations, a student must provide diagnostic evaluation by a licensed psychologist, psychiatrist or a medical provider. Please note that this form does not guarantee accommodations but will be considered along with the appropriate documentation. Please return this form along with the appropriate documentation by June 1st, 2008.

Wellness Center
The Wellness Center offers medical and mental health services to all Carleton students. Explore their site to find out about how to access care and the variety of services offered.

Recreation Center
Whether you want a physically challenging activity or a space for quiet contemplation, the Recreation Center is the place to be.

Religious Preference Form
If you wish to get involved in religious life at Carleton or in the Northfield community, this form will help us put you in touch with the appropriate person, group or church.

Office of the Chaplain
The role of the Carleton Chaplaincy is to help deepen the spiritual and ethical life of the campus. Chaplains also serve as resources for student religious groups of all faiths on campus.
