New Student Information
Welcome Carleton Class of 2016!
As part of our effort to provide quality health care to Carleton students, we ask that you take time to complete two required items by August 15th:
1) Enter your Immunization Information into our Online Student Health portal (link here), which inputs them directly into your confidential electronic health record.
2) Complete the Tuberculosis (TB) Risk Assessment Form using the Online Student Health portal.
Instructions on how to submit Immunizations and Tuberculosis (TB) Risk Assessment (coming soon).
Please read the following before submitting your information:
All incoming Carleton students are REQUIRED to:
- Submit dates of immunization against measles, mumps and rubella (MMR), for which you will need evidence of 2 doses.
- Submit dates of immunization against tetanus and diphtheria (Tdap preferred, Td also acceptable); must have been received within the past 10 years.
- Complete the online Tuberculosis (TB) Risk Assessment form and, if indicated, submit hardcopy results of a Mantoux skin test or IGRA blood test performed within the past 6 months.
Please note: If you do not complete your Immunization Information and the Tuberculosis (TB) Risk Assessment Form online or if you do not comply with all the required immunizations, you will not be allowed to register for winter term classes.
Note to International students: If possible, complete the required immunizations and TB testing before leaving your country. Immunizations and other medical treatments in the United States may cost more than what you may pay in your home country. The Carleton Student Injury & Sickness Plan covers required immunizations. It does not cover a mantoux skin test or IGRA blood test and it does not cover chest x-rays if you have a positive mantoux skin test or IGRA blood test. If possible, complete these services in your home country before coming to Carleton.
Flu shots and other immunizations are available on-campus multiple times throughout the year, beginning fall term. Check our immunization webpage for specific dates.

During the summer months (June 18-August 10), Student Health and Counseling is only open two days per week. We will resume regular Monday-Friday hours on August 13, 2012.
