How to Use
Token (Hiawathaland Transit Bus)
- Complete a Medical Token/Voucher Request form
- Pick up your token at the Info Desk in Sayles-Hill
- Present token to Hiawathaland Transit bus driver (bus routes and details here)
- Dial-A-Ride appointment instructions here
Voucher (First Choice Shuttle)- Complete a Medical Token/Voucher Request form
- Make an appointment with First Choice Shuttle 507-645-4447
- Pick up your voucher at the Info Desk in Sayles-Hill
- Document travel details on back side of voucher
- Present voucher to First Choice Shuttle driver
