Health Assessments for Carleton Programs

Students on Carleton programs and exchange programs such as NUS are required to complete a Health Assessment Form. The form can be accessed here. Health Assessments may be done by any licensed medical provider. Most students see their home doctor, or go to the local Allina Clinic or Family Health Medical Clinic. A special health assessment clinic is held at Carleton College each term to which eligible students will receive an invitation to schedule an appointment. 

The Health Assessment form is due the third Friday of the term prior to the program. For 2013-14 the deadlines are:

Friday, October 4, 2013 for winter break 2013 and winter term 2014 programs

Friday, January 24, 2014 for spring break and spring term 2014 programs

Friday, April 18, 2014 for summer/fall 2014 and academic-year 2014-15

Most non-Carleton programs also have a health screening process. Look for details in your acceptance packet.
